Living with a long-term health condition can be overwhelming — but you don’t have to manage it alone. At Googong Family Practice, our Chronic Disease Management (CDM) Plans are designed to support patients with ongoing medical needs through structured, collaborative, and proactive care.

If you’ve been living with a chronic condition for more than six months — such as diabetes, arthritis, heart disease, asthma, or chronic pain — you may be eligible for a personalised management plan under Medicare.

Our GPs will work closely with you to develop a tailored health plan that puts your wellbeing first and gives you access to allied health support like physiotherapy, dietetics, podiatry, and more — often with Medicare-subsidised visits.
What Is a Chronic Disease Management Plan?
A CDM Plan is a GP-led strategy that outlines your condition, treatment goals, healthcare team, and ongoing care strategies. It helps ensure that you’re getting the right care at the right time — across multiple providers — while staying in control of your health.
There are two key types of plans:

GP Management Plan (GPMP): A comprehensive plan for patients with one or more chronic medical conditions.

Team Care Arrangements (TCA): Coordinated care between your GP and at least two other allied health professionals (e.g. a physio, dietitian, or psychologist).
Together, these plans may provide access to up to five Medicare-rebated allied health visits per calendar year.

Who Is Eligible?
You may be eligible for a CDM Plan if you’ve been diagnosed with a medical condition that has lasted or is expected to last at least six months. This may include (but is not limited to):
